Summary of Position

The Registered Nurse is responsible for working with healthcare professionals to help treat patients with various injuries, illnesses, or disabilities. The Registered Nurse plays a critical role in the delivery of quality medical care with in the center. This position requires a person with strong clinical skills along with assessment and communication skills. The Registered Nurse provides care as described by the established policies and procedures of RCCHC. This position will provide clinical care in collaboration with other partnerships to achieve positive patient outcomes. The Registered Nurse will model behaviors consistent with the published values and the Code of Conduct of RCCHC.

Roanoke Chowan Community Health Center will provide open access, community-oriented primary care of the highest quality to all residents within our service area and surrounding counties, regardless of their ability to pay.

Minimum Qualifications

 

* Must demonstrate competencies in nursing practice and utilization of the nursing process. * Must demonstrate the knowledge and skill necessary to provide clinical care. * Must demonstrate knowledge of the principles of growth and development over the lifespan, and possesses the ability to assess data reflective of the patient’s status, and interpret the appropriate information required to identify needs of the patient relative to his/her age and condition. * Must have effective communication, initiative, problem-solving ability, accountability flexibility, stress management, and basic computer skills. * Must be flexible and willing to work at different locations as needed. * Must have computer skills. * Must be able to work with all patient populations. * Works effectively with others and is able to organize workload, as well as assume responsibility and maintain confidentiality. * Must be able to work independently, exercise appropriate decision-making skills, and function effectively on a team. * Must be able to speak and communicate clearly and effectively.

Education: Graduate from an accredited school of nursing required

Experience: Work experience in a medical practice preferred

Certification(s)/Licensure: BLS certification required

Knowledge: 

* Knowledge and thorough understanding of Federally Qualified Health Centers (FQHC’s) in regard to the Federal 330 Public Health Service Act  Knowledge of thorough understanding of the 340B program * Knowledge of computer systems and applications.

Skills: 

* Skill in exercising a high degree of initiative, judgment, and discretion. * Skill in analyzing situations accurately and taking effective action. * Skill in establishing and maintaining effective working relationships with the medical and administrative staff, patients and the community. * Skill in planning, organizing, and prioritizing work; delegating and achieving goals and objectives. * Skill in exercising judgment and discretion in developing, interpreting, and implementing departmental policies and procedures. * Skill in identifying and resolving problems.

* Ability to plan, organize, and integrate priorities and deadlines. * Ability to identify, analyze, and interpret complex data and, resolve operational problems. * Ability to evaluate and make recommendations for continuous quality improvement. * Ability to evaluate and make recommendations for continuous risk assessment. * Ability to anticipate and react calmly in emergency situations. * Ability to communicate clearly and effectively orally and in writing. * Ability to competently use Microsoft Office, including Word, PowerPoint, Excel, and appropriate organizational software.

Essential Functions

* Prioritize nursing actions including reflecting patient needs, acuity, and desired outcomes  Implement physician orders accurately within appropriate time frame. * Provide patient care based on scientific principles/nursing theory and evidence based practice. * Reviews data collection and assesses patient complaint and request for service. * Evaluates provider schedule and determines appointment vacancies. * Collaborate with nurses (RNs and LPNs) using critical thinking in clinical problem-solving and decision-making processes relative to scope of practice, knowledge, competency, and experience * Perform independent nursing functions using RCCHC established procedures, policies, and guidelines. * Provide nursing intake, triage, assessment and education to patients via telephone, face to face interactions, and secure messaging. * Administer medications/immunizations as ordered by RCCHC providers, documenting accurately and observing patients for reactions * Accurately execute technical procedure when ordered such as EKG’s, drawing blood, obtaining and preparing laboratory specimens * Complete paperwork for assigned providers as per RCCHC guidelines * Collaborates with provider or refers to Intake Guidelines to determine appropriate point of service. * Conveys information and coordinates follow up action plans at request of provider.  Delivers patient care and performs treatments and procedures in accordance with agency’s established policy and procedures. * Delegates tasks to lab, nursing, and clerical staff judiciously. * Collaborates with all staff members to enhance positive patient outcomes. * Makes empowered decisions in response to patient needs. * Communicates effectively, presenting positive and professional image in all interactions. * Assists with precepting students and/or new employees * Adheres to organizational and departmental policies and procedures. * Maintains a clean and safe environment. * Knowledgeable of and enforces RCCHC safety policies and workplace safety rules. * Participates in achievement of organizational goals. * Applies RCCHC mission and values to customer service and care delivery. * Maintain compliance with workflows, policies and procedures as well as other activities to support PCMH model of care and accreditation
