Summary:
The Licensed Practical Nurse (LPN) is responsible for delivering high-quality nursing care to residents under the supervision of the Nurse Practitioner. The LPN works collaboratively with the healthcare team to ensure the well-being and comfort of residents.
Education Requirement:
* Complete a high school diploma or GED and * Completion of an accredited practical nursing program
Experience Requirement:
* One (1) year experience Preferred * New Graduates are welcome
Licensure/Certification Requirement:
* Pass the National Council Licensure Examination (NCLEX-PN) * Complete a state-approved accredited program (Compact State License) * Current CPR/BLS card
Key Job Responsibilities:
* Practice within the scope of practice of an LPN * Complete all documentation by the end of shift * Communicate with patients, their families, and other caregivers * Educate patients and families on mental illness, medications prescribed, importance of adherence to maintain stabilization, and possible side effects of medications * Communicate with members of the mental health treatment team (peer specialists, mental health techs, clinicians, NP/PA/MD/DO) and other outside healthcare providers (Primary Care Providers-PCP) in an effort to coordinate efficient and effective care for the patients * Schedule mental health and medical appointments * Participate in the development of treatment and housing plans for patients in collaboration with a Registered Nurse (RN), Nurse Practitioner (NP), Physician Assistant (PA), Doctor of Osteopathy (DO), or Medical Doctor (MD) * Complete mental status exam and behaviors that includes an ongoing nursing evaluation of patient’s condition and document accordingly * Evaluate the effectiveness of medications, monitor for any adverse reactions, and immediately report any changes to the NP or MD * Monitor patients in the self-administration of medications * Administer medications to patients who are unable to perform self-administration according to label instructions * Administer medication via oral, subcutaneous, intramuscular, sublingual, intradermal routes * Participate in treatment planning and activities the housing facility and its residents * Responsible for supervision of residents and resident activities * Assist Housing Manager with monitoring and maintaining activities related to daily activities of the resident and facility (shopping, cleaning, maintaining inventory, meal planning, and assigning household chores) * Assist Housing Manager with assuring the house meets all regulatory standards * Evaluate the progress of patients in treatment and housing * Maintain patient medical records * Administer medications ordered by a Medical Doctor (MD) or NP * Other duties as assigned within the scope of practice of the LPN
