The Role We're looking for a Registered Nurse (RN) - Virtual to provide remote clinical oversight, telephonic triage, and care coordination. In this 100% remote role, you will perform clinical assessments, monitor patient treatment plans, and collaborate with physicians and interdisciplinary teams to ensure quality care and regulatory compliance.
What You'll Do Conduct virtual clinical intakes, health screenings, and wellness checks with patients and caregivers. Perform telephonic triage and clinical assessments for patient calls, directing care using established protocols. Review and update care plans in alignment with physician orders and state regulations. Monitor Remote Patient Monitoring (RPM) data and vital trends, flagging clinical changes for the care team. Serve as a primary virtual point of contact for patients, families, attending physicians, and home care staff. Maintain accurate, timely clinical documentation in our EHR platforms. Participate in virtual case conferences, team huddles, and quality assurance reviews.
What You Bring Active, unencumbered Registered Nurse (RN) license in the state of New Jersey required (Multi-state Compact Licensure preferred). Minimum of 1–2 years of clinical experience in home health, triage, or remote care management. Strong clinical triage, assessment, and critical-thinking skills. Professional, empathetic communication skills across phone and video channels. Proficiency with Google Workspace and EHR care management systems. Dedicated home office setup with high-speed internet meeting all HIPAA privacy standards.
