Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Licensed Practical Nurse (LPN) Coordinator in Beverly, Massachusetts.
POSITION OVERVIEW • PACE-focused LPN coordination role supporting older adults within outpatient and home-based care environment • Opportunity to build long-term relationships while coordinating care and supporting patient independence • Primarily clinic-based role with occasional home visits as needed • Collaborative interdisciplinary environment working closely with Occupational Therapy and nursing teams • Autonomous role assessing patient needs and recommending care plan updates

SCHEDULE • Monday-Friday • 8:00 AM-4:00 PM schedule • No weekends • No call responsibilities • Primarily outpatient with occasional home visits

ROLE RESPONSIBILITIES • Coordinate home care and outpatient support services for geriatric patient population • Assess patient medical and functional changes • Recommend updates to patient care plans • Collaborate with Occupational Therapy and interdisciplinary care teams • Support continuity of care and patient independence goals • Conduct home visits as needed within care coordination structure
