This is an hourly position working PRN. Clients will be assigned based on your home geographic area. Job Summary The Medical Care Nurse Adivsor provides compassionate, trauma-informed, and family-centered care in alignment with the Texas Child-Centered Care (T3C) Blueprint (https://www.dfps.texas.gov/Texas_Child_Centered_Care/T3C_System_Blueprint.asp) and the agency’s credentialed treatment models. The RN ensures that assigned children, youth, and families receive individualized, developmentally appropriate, and medically sound services that support optimal physical health outcomes. The RN plays a critical role in supporting CK Case Managers and caregivers managing primary medical needs and complex medical needs children. General Responsibilities Conduct comprehensive nursing assessments for assigned children and youth, integrating medical needs. Collaborate with interdisciplinary teams for assigned children to ensure care aligns with physicians’ orders, evidence-based practices, and the agency’s treatment model(s). This position will be a part of the child’s treatment team. Be available by phone to support on-call case managers after hours to guide appropriate medical intervention as needed. Support transitions of care, including hospital discharges, placement changes, and reunifications, with continuity and safety as priorities for assigned children. Provide consultation, training, and direct support to staff and caregivers working with children who have complex medical needs in coordination with care plans as needed. Collaborate with foster, biological, and adoptive families to promote understanding of children’s medical care requirements for assigned children. Maintain complete and compliant documentation in alignment with regulatory, contractual, and agency. Maintain current licensure and knowledge of pediatric, trauma-informed, and family-centered care standards. Participate in professional development and continuing education relevant to child welfare nursing, complex medical care, developmental disabilities, and maternal child health. Collaboration and consultation during admissions as needed. Qualifications Education: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing accepted with commitment to continuing education. Licensure: Active, unrestricted Registered Nurse (RN) license in the State of Texas. Experience: Minimum 2–3 years in pediatric care. Home health experience helpful. Experience with children in DFPS or similar systems preferred. Knowledge and Skills: Trauma-informed, culturally responsive, and child-centered care principles; Pediatric and adolescent health; Understanding of Complex Medical Needs, IDD, ASD, and Pregnant and Parenting Youth care packages; Medication safety; Crisis intervention; Human trafficking awareness. Other Requirements: Valid driver’s license and reliable transportation; Ability to travel; Strong interpersonal, communication, and organizational skills.
