SCMHCC is seeking a dedicated Adult Case Manager to deliver high-quality, medically necessary services within our Community Support Program in El Dorado. In this role you will partner with adults living with serious mental illness and co-occurring substance use disorders, guiding their recovery using the Strengths Model of Case Management and IDDT evidence-based practices. This is a meaningful, community-based position for someone passionate about mental health recovery and client empowerment.

WHAT YOU'LL DO

* Develop continuous, supportive working relationships with assigned clients in the Community Support Program. * Conduct Strengths Assessments, co-develop Personal Recovery Plans, and facilitate individualized Crisis Plans. * Deliver the majority of direct client services in community and non-office settings. * Review treatment plans quarterly to monitor progress, update goals, and ensure quality of care. * Apply IDDT model-specific interventions including supportive counseling, solution-focused techniques, and behavioral analysis. * Help clients understand symptoms of mental illness and co-occurring substance use disorders and minimize their impact across life domains. * Facilitate client autonomy by building on individual strengths, increasing community integration, and supporting symptom management. * Assist clients in building natural support networks and connecting with community agencies and resources. * Serve as a client advocate and assist with integration into emergency service systems when needed. * Collaborate with multidisciplinary service providers and make appropriate referrals. * Complete accurate, timely progress notes documenting interventions for all billed codes. * Maintain reliable and predictable attendance — an essential function of this role. * Perform other duties as directed by the Team Lead or Community Support Services Director.

QUALIFICATIONS & REQUIREMENTS

EDUCATION

BA/BS in human services, psychology, social work, or related field — or equivalent combination of education and experience. One year of experience may substitute for one year of education.

EXPERIENCE

Demonstrated interpersonal skills and ability to work with individuals with serious emotional disturbances. Ability to respond effectively in a wide range of human services situations.

TRANSPORTATION

Reliable personal transportation required. Must have the ability to transport clients as needed. Must meet agency driver's license and insurance requirements.

BACKGROUND

Must pass criminal background check and driver's license review to SCMHCC standards prior to hire.
