Job Description: What You’ll Do: The Hospice LPN Team Resource performs professional nursing care, functions as a member of the hospice team and provides input into the assessment, planning, and evaluation of nursing services. Under the direction and supervision of the RN Case Manager - Hospice, the LPN is accountable for the quality of nursing care he or she provides and assumes responsibility for the provision of nursing care in accordance with the established plan of care for assigned patients. More Specifically: Performs nursing visits to patients as delegated/assigned by the Hospice RN Case Manager/Clinical Manager. Provides hospice nursing care to patients and families utilizing current principles of palliative care and symptom management to produce hospice outcomes as directed by the RN. Observes, records, and reports significant changes in the patient’s healthcare status to the MD and RN Case Manager. Communicates with the physician to report significant changes in the patient’s condition; obtains/receives physician’s orders as required. Participates in the weekend and holiday rotation as defined by Beth Israel Lahey Health at Home Provide documentation of services utilizing problem-oriented record; Participate in bi-weekly hospice case conference or as required. What You’ll Need: Graduate from a Licensed Practical Nursing program accredited by the National League of Nursing (NLNAC) Current licensure to practice as an LPN in the state of Massachusetts Maintains AHA BLS certification Two years of experience as a Licensed Practical Nurse in an acute care, rehab or home care setting. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
