Job Summary

The Physical Therapist evaluates, plans, and administers therapeutic procedures in physical medicine and rehabilitation as prescribed by a referring physician. This role establishes individualized physical therapy goals, provides treatments, and regularly reassesses patient progress, documenting all findings accurately. 

Schedule: PRN, as needed, schedule varies

Qualifications

* Doctorate in Physical Therapy (DPT) required * 0-2 years of experience in a physical therapy setting required

Licenses and Certifications

* PT - Physical Therapist license in the state of employment required * BCLS - Basic Life Support within 30 days of hire required

Essential Functions

* Conducts thorough physical therapy evaluations to assess patient needs and develops individualized treatment plans. * Administers safe and effective therapy treatments, ensuring all interventions are appropriately documented. * Reassesses and adjusts treatment plans within appropriate timeframes to align with patient progress and changing needs. * Provides clear, concise, and accurate documentation of evaluations, treatment plans, progress notes, and billing information. * Educates patients and their families on treatment plans, therapeutic exercises, and post-treatment care to support long-term recovery. * Operates therapy equipment safely and maintains a clean, organized, and safe environment for patients and staff. * Collaborates with referring physicians, peers, and interdisciplinary team members to ensure continuity and quality of patient care. * Demonstrates population-appropriate competencies for all relevant patient demographics, tailoring care to individual needs. * Maintains compliance with facility policies, including safety, infection control, and risk management procedures. * Adapts to changes professionally and effectively, following the chain of command to address and resolve issues impacting patient care. * Performs other duties as assigned.
