Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Registered Nurse (RN) Case Manager in Lynn, Massachusetts.

POSITION OVERVIEW • Hybrid RN Case Manager role focused on coordinating care for aging adults living independently at home • Community-based nursing opportunity centered around care planning, continuity of care, and service coordination • No bedside nursing responsibilities required • Hybrid structure combining community patient visits with remote follow-up and care coordination work • Mission-driven senior care environment with strong interdisciplinary collaboration

SCHEDULE • Monday-Friday schedule • 8:00 AM-4:00 PM hours • Three community-based days weekly • Two remote workdays weekly • No nights • No weekends • No call responsibilities

ROLE RESPONSIBILITIES • Develop individualized patient care plans • Coordinate community-based services and ongoing care needs • Conduct patient visits within community and home settings • Manage follow-up and care coordination remotely • Collaborate closely with interdisciplinary care teams • Support aging adults in maintaining safe independent living

Requirements • Active Massachusetts RN license REQUIRED • Minimum 2 years RN experience REQUIRED • Home care experience preferred • Strong communication and organizational skills REQUIRED • Comfortable working independently within community settings REQUIRED • Team-based care approach REQUIRED • Comfortable with hybrid remote/community workflow REQUIRED
