Position Overview

The NJ -Nurse Manager provides leadership, oversight, and support to a team of traveling Registered Nurses delivering care in patient homes and community-based settings. This role ensures clinical excellence, operational efficiency, and compassionate patient-centered care aligned with Ponos Care’s mission and clinical standards.

The Nurse Manager collaborates closely with clinical leadership to ensure nursing staff are supported, care delivery is coordinated effectively, and patient outcomes reflect the highest standards of quality and safety. This role also ensures compliance with regulatory requirements and promotes continuous improvement in clinical practice.

Core Responsibilities

Clinical Operations Leadership

* Manage day-to-day operations of traveling RN team supporting home-based patient care * Coordinate nursing schedules, assignments, and patient visits to ensure optimal care delivery * Monitor clinical workflows and ensure timely documentation in EMR systems * Support implementation of care delivery protocols and treatment plans

Team Supervision & Development

* Provide leadership, coaching, and mentorship to nursing staff * Conduct performance evaluations and provide ongoing feedback * Promote professional development and clinical competency improvement * Foster a collaborative and supportive work environment

Patient Care Quality & Outcomes

* Ensure delivery of high-quality, culturally competent, and patient-centered care * Monitor patient care plans and assist with complex case management as needed * Collaborate with interdisciplinary teams to improve patient outcomes * Support continuous improvement of care coordination practices

Care Coordination & Scheduling

* Coordinate patient assignments to ensure efficient utilization of clinical resources * Oversee home visit schedules across multiple geographic areas * Ensure appropriate staffing coverage for patient care needs * Address scheduling challenges and adjust resources as needed

Policy Implementation & Compliance

* Support development and implementation of patient care programs, policies, and procedures * Ensure compliance with clinical regulations and healthcare standards * Monitor adherence to documentation standards and quality measures * Escalate clinical or operational risks to leadership as appropriate

Collaboration & Communication

* Partner with clinical leadership to identify challenges and opportunities for improvement * Communicate operational and clinical updates to Director of Clinical Services and CNO * Collaborate with interdisciplinary care teams to ensure coordinated care delivery * Promote a culture of respect, inclusion, and professionalism

 

Program Leadership & Strategy: 

* Ensures nursing program supports home-based care model * Aligns nursing services with Ponos Care’s value-based care approach * Develops care delivery strategies that improve patient outcomes * Supports initiatives that reduce hospital readmissions

Regulatory & Compliance Oversight: 

* Ensures nurses follow clinical documentation standards * Maintains HIPAA compliance * Ensures nursing staff follow scope of practice rules * Ensures proper certifications and licensure are current

Operational Excellence & Performance Management:

* Ensures nursing schedules are efficient * Reduces missed patient visits * Improves care coordination processes * Monitors nurse productivity and workload balance

Team Leadership & Development:

* Provides coaching to RNs * Conducts performance reviews * Supports training and clinical development * Improves nurse retention and engagement

Innovation & Growth:

* Implements telehealth workflows * Supports new clinical programs * Identifies opportunities to improve patient care delivery * Contributes ideas to improve care coordination model

 

QUALIFICATIONS AND EDUCATION

* Bachelor’s degree in Nursing or related field * Active Registered Nurse (RN) license in applicable state(s) * Current certifications such as BLS, ACLS, CPR (as applicable) * 3+ years clinical experience in acute care, infusion therapy, pain management, hematology, or community health * 1+ years of nursing leadership or supervisory experience * Experience coordinating care in home health or mobile healthcare environments preferred * Ability to travel to patient locations as needed
