We are seeking a highly skilled Physical Therapist to join our team as a PT for Home Health. Minimum Qualifications:
* Current state license as a Physical Therapist * Minimum of 1 year of experience in home health * Excellent communication and interpersonal skills * Ability to work independently and as part of a team * Strong problem-solving and critical thinking skills
Responsibilities:
* Conducting patient evaluations and developing individualized treatment plans * Providing hands-on therapy to patients in their homes * Educating patients and their families on proper techniques and exercises * Collaborating with physicians, nurses, and other healthcare professionals to ensure the best possible patient outcomes * Maintaining accurate and up-to-date patient records
