We are looking for a Hospice Liaison to work Full Time 40 hours weekly, in this position, you will be responsible for new referrals to the Agency in the south shore. Serve as the primary liaison to prospective patients and designated health care providers as well as insurance case management professionals and community groups to ensure safe and appropriate care for all new patients. Compensation: $75,000- 90,000 (based on experience and credentials) plus bonus! Duties and Responsibilities: Develops, cultivates, and maintains strong professional relationships with physicians, hospitals, skilled nursing facilities, rehabilitation centers, social workers, case managers, discharge planners, referral sources, patients, and families to generate new referral opportunities and drive admissions growth. Actively identifies, develops, and converts new referral sources while strategically growing existing accounts. Creates and executes outreach strategies designed to increase referrals, admissions, market share, and overall Agency census. Serves as a key business development liaison between referral partners, patients, families, and Agency leadership, ensuring a seamless referral and admission experience while positioning the Agency as a trusted provider of high quality care. Educates referral sources, patients, and families on the Agency’s services, clinical capabilities, differentiators, and value proposition. Effectively communicates how the Agency can meet patient needs and support positive outcomes. Proactively conducts sales calls, facility visits, physician office visits, community outreach, presentations, networking events, and other business development activities to establish new relationships and strengthen existing partnerships. Develops and maintains a strategic referral network by identifying high-value accounts, understanding referral patterns, uncovering opportunities, and creating individualized relationship management strategies to maximize referral potential. Partners closely with Intake and clinical teams to assess referral opportunities, determine admission appropriateness, and coordinate timely admissions based on patient needs, Agency criteria, and daily operational capacity. Consults with referral sources regarding complex patient discharges, barriers to placement, and appropriate care options, positioning the Agency as a responsive and solutions oriented partner. Maintains consistent communication and follow up with referral sources throughout the referral and admission process to ensure exceptional customer service, strengthen relationships, and encourage repeat referrals. Responds promptly and professionally to questions, concerns, complaints, and inquiries from referral partners, patients, families, and community providers, using each interaction as an opportunity to strengthen relationships and reinforce the Agency’s reputation. Tracks and analyzes referral activity, admissions, payor mix, referral source performance, conversion opportunities, and account activity to identify trends, measure performance, and develop strategies for continued growth. Reviews weekly Croi Health Admission Reports and maintains accurate business development statistics, including referral sources, admissions, insurers, conversion activity, and account performance. Uses data to prioritize sales efforts and develop targeted growth strategies. Establishes measurable goals for referral growth and account development and consistently works toward achieving or exceeding assigned census, admission, and business development targets. Participates in professional associations, community organizations, healthcare networking events, educational programs, and outreach initiatives to increase brand awareness and expand the Agency’s referral network. Collaborates with Agency leadership and interdisciplinary teams to identify market opportunities, address referral barriers, improve customer experience, and develop initiatives that support sustainable census and revenue growth. Maintains a strong understanding of the competitive healthcare landscape, referral trends, payer requirements, and community resources to effectively position the Agency and differentiate its services within the market. Represents the Agency with professionalism, enthusiasm, and a results oriented mindset, consistently seeking opportunities to expand the referral base, strengthen strategic partnerships, and drive admissions growth. Experience/Education: Bachelor’s degree preferred. 5+ years of proven sales or business development experience. Strong track record of generating new business and exceeding sales goals. Excellent relationship-building, networking, and communication skills. Self-motivated, persistent, and comfortable with prospecting, cold calling, and face-to-face sales. Healthcare or referral-based sales experience preferred.
