Overview Registered Nurse – Case Management in Hermiston, OR. This travel assignment is an inpatient case management role within a Critical Access Hospital. Start date 06/08/2026 for 13 weeks. Guaranteed 40 hours per week with a 5x8 Days shift. Weekly pay range is $3,749–$3,919.

Responsibilities - Assess patients’ healthcare needs and develop a comprehensive, interdisciplinary care plan with the patient and family. - Facilitate communication among patients, families, and healthcare providers; coordinate transitions between care settings and plan for post-hospital care. - Evaluate medical necessity for treatments and hospital stays; collaborate with insurers to obtain authorizations and identify discharge barriers. - Advocate for patients’ needs and preferences; educate patients and families about diagnoses, treatments, and self-care; connect with community resources. - Maintain thorough documentation of assessments, interventions, and outcomes; participate in interdisciplinary care conferences and coordinate plan implementation across disciplines.
Requirements - Active Oregon RN license - CRASE training - Education: Graduate of an accredited nursing program or related medical occupation (BSN/MSN preferred) - Experience: Previous clinical experience in an acute care or home health setting; discharge planning/case management experience preferred - Language: Bilingual (English/Spanish) preferred - Ability to work in an inpatient setting and collaborate with a wide healthcare team
