Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Registered Nurse (RN) Case Manager in Clewiston, Florida.
POSITION OVERVIEW • Community-based RN Case Manager role focused on patient advocacy, care planning, and coordination of complex medical needs • Opportunity to conduct assessments within home and clinic settings while supporting long-term patient outcomes • Outpatient nursing environment centered around care coordination and interdisciplinary collaboration • Manageable caseload allowing focused and relationship-driven patient support • Role supporting diverse patient populations within community healthcare setting

SCHEDULE • Monday-Friday daytime schedule • Outpatient and community-based care coordination • Telephone-only call rotation every 4-6 weeks • No weekend clinic burden • Caseload range: 8-13 patients
