SCHEDULE & COVERAGE
This is a full-time, driving position serving Wooster and surrounding communities.
Schedule includes:
* Three weekdays: 4:00 PM – 12:00 AM * Every other weekend: 8:00 AM – 8:00 PM * Participation in a rotating holiday schedule is required.
WHAT YOU'LL DO
As an LPN on our Individualized Care team, you'll provide skilled nursing care under the direction of a Registered Nurse while helping patients remain comfortable in their homes or care facilities.
Responsibilities include:
* Deliver compassionate, patient-centered nursing care in home and facility settings. * Monitor patient conditions and report changes to the supervising RN. * Administer treatments and medications as directed. * Accurately document care using electronic medical records. * Educate and support patients and families throughout their hospice journey. * Collaborate with an interdisciplinary care team to ensure each patient receives individualized care.
WHAT WE'RE LOOKING FOR
Qualified candidates will have:
* Current Ohio Licensed Practical Nurse (LPN) license in good standing. * At least one year of nursing experience preferred. * Current CPR certification. * Basic computer skills for electronic documentation. * A valid driver's license, reliable transportation, and state-required automobile insurance. * Ability to travel throughout the service area in various weather conditions. * Successful completion of a criminal background check and 10-panel drug screening.
