Department: 34100 South Metro C - Nursing Status: Full time Benefits Eligible: Yes Hours Per Week: 40 Schedule Details/Additional Information: Service areas: Calumet City, Lynwood, Dolton, Glenwood, Lansing, Posen, South Holland, Thornton, Burnham, Riverdale Pay Range $26.10 - $39.15 Home Health Physical Therapist Assistant (PTA) Schedule: Full-time | Day shift (with weekend/holiday rotation as needed) Why Join Us Provide meaningful, one-on-one care in the home setting Work closely with a supportive interdisciplinary team Build strong patient relationships and see real impact Autonomy with strong therapist support and collaboration Role Overview The Home Health PTA works under the direction of a Physical Therapist to provide high-quality, patient-centered care in the home. This role focuses on delivering individualized therapy interventions, supporting care coordination, and ensuring excellent patient outcomes. Key Responsibilities Patient-Centered Care Deliver individualized treatment plans established by the supervising Physical Therapist Build strong, compassionate relationships with patients and families Adapt communication style to meet patient and family needs Promote patient independence with self-care, disease management, and safety Involve patients and families in care planning and treatment progression Quality & Clinical Excellence Provide evidence-based therapy interventions to achieve patient goals Identify changes in patient condition and communicate with supervising therapist Maintain accurate, timely documentation to meet regulatory and reimbursement standards Participate in quality improvement initiatives and promote patient safety Care Coordination Collaborate with the interdisciplinary team to support coordinated care Communicate effectively using tools like SBAR to ensure continuity Assist in discharge planning and transition of care Participate in case conferences and team discussions Professional Development & Teamwork Engage in ongoing education and training opportunities Maintain current knowledge and clinical competency Support a collaborative, respectful team environment Demonstrate strong communication, feedback, and problem-solving skills Operational Support Adhere to organizational policies and productivity expectations Efficiently manage time, travel, and patient caseload Utilize resources appropriately to support optimal outcomes Education & Experience Graduate of an accredited Physical Therapist Assistant program Minimum 2 years of clinical experience (or strong clinical affiliation experience) Meets all state continuing education requirements Licensure & Certifications Licensed Physical Therapist Assistant in the State of Illinois (or eligible) BLS certification (or obtained within 6 months) Valid driver’s license Skills & Qualifications Strong clinical and technical home health therapy skills Excellent communication and documentation abilities Strong time management and organizational skills Ability to work independently while collaborating with supervising therapist Proficiency with home therapy equipment and assistive devices Work Environment & Physical Requirements Travel within assigned territory, including varying weather and road conditions Provide care in patient homes and assisted living settings Ability to lift up to 35 lbs (with equipment/assistance as needed) Physical activity including bending, climbing stairs, lifting, and patient transfers Our Commitment to You: Advocate Health offers a comprehensive suite of Total Rewards: benefits and well-being programs, competitive compensation, generous retirement offerings, programs that invest in your career development and so much more – so you can live fully at and away from work, including: Compensation Base compensation listed within the listed pay range based on factors such as qualifications, skills, relevant experience, and/or training Premium pay such as shift, on call, and more based on a teammate's job Incentive pay for select positions Opportunity for annual increases based on performance Benefits and more Paid Time Off programs Health and welfare benefits such as medical, dental, vision, life, and Short- and Long-Term Disability Flexible Spending Accounts for eligible health care and dependent care expenses Family benefits such as adoption assistance and paid parental leave Defined contribution retirement plans with employer match and other financial wellness programs Educational Assistance Program Note: Eligibility for programs listed above may depend on your FTE or status (e.g., full-time, part-time, per diem, temporary, etc.); please ask a Recruiter for more information during an interview.
