About this role
SUMMARY:
The Case Manager provides supportive services to high-risk individuals through assessments, individualized planning, and service coordination. This role ensures participants access to housing, employment, behavioral health, and trauma recovery support.
JOB RESPONSIBILITIES:
* Complete intakes and eligibility documentation. * Develop and implement individual case plans with measurable goals. * Maintain a caseload of up to 25 participants. * Provide referrals to behavioral health, housing, job training, and legal resources. * Track outcomes and update participant files with progress notes. * Attend interdisciplinary team meetings and case conferences. * All other duties as assigned.
Education, Experience, and Skills Required:
* Education: Bachelor’s degree in Social Work, Psychology, Human Services, or related field. * Experience: Two years of experience in case management or violence prevention work. * Skills: * Strong documentation and time management skills. * Effective communication and interpersonal abilities. * Trauma-informed and culturally competent approach.

This role is only available to Illinois residents.
