PART-TIME PHYSICAL THERAPIST – FLEXIBLE, REWARDING OPPORTUNITY (MIN. 15 VISITS/WEEK)

WHAT YOU’LL DO
* Perform comprehensive patient evaluations and develop individualized treatment plans * Deliver hands-on, goal-driven physical therapy to improve mobility, strength, and function * Track patient progress and adjust care plans to maximize outcomes * Educate patients and families to support recovery and long-term success * Maintain timely and accurate clinical documentation * Collaborate with healthcare team members to ensure coordinated, high-quality care
WHAT WE’RE LOOKING FOR
* Licensed Physical Therapist (PT) in good standing * Strong clinical skills with a patient-first mindset * Excellent communication and documentation abilities * Ability to work independently while contributing to a collaborative team * Commitment to quality care and positive patient outcomes

Requirements
* Successful completion of accredited physical therapy program and state licensure exam.
* Valid Physical Therapy license
* 1 year of home health experience preferred
* CPR certification
* Valid Driver’s license
* Valid Auto Insurance
* HomeCare HomeBase experience preferred

* Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
* Must be able to lift, carry, pull, or push up to 100 lbs..
