YOUR DAY-TO-DAY LIFE AT CUSTOM:
As a Physical Therapist at Custom Home Health, you will:
* Deliver individualized care that empowers patients to achieve their health and independence goals. * Collaborate with RNs and interdisciplinary teams to design and implement personalized care plans. * Educate patients and their families on therapy techniques, exercises, and treatments to promote lasting well-being. * Coordinate and administer treatments that enhance physical function and overall health. * Approach every task with compassion, professionalism, and a commitment to excellence.

WHAT WE’RE LOOKING FOR:
* Licensed Physical Therapist: Must hold a valid Physical Therapy license. * At Least 1 year of Home Health Experience with OASIS knowledge * Passion for Excellence: A genuine desire to provide top-tier care to our patients and community. * Team Player: Strong collaboration and communication skills.
