Monday - Friday, 12:00PM - 8:00PM OR Tuesday - Friday 12:00PM - 8:00PM and Saturday 9:00AM - 5:00PM
Job Summary
Pilsen Wellness Center is seeking a dedicated and compassionate Case Manager to participate in the implementation of the individual’s service plan, specifically designed to increase an individual’s skills in independence in daily living, economic self-sufficiency and community integration.
Responsibilities
* Assists the client in achieving independence (e.g., housing, finances) * Assists with supported employment/education as needed * Maintain timely and accurate clinical documentation * Helps participants to access the services specified in their service plans * Adhere to ethical and confidentiality standards
Qualifications
* High School Diploma or GED * Active Drivers License * Bilingual (English/Spanish) preferred * 3-5 years of administrative and clerical assistance experience * Cultural humility and the ability to build trust with diverse populations * Strong documentation and collaboration skills
Compensation & Benefits
