To learn more about Macomb County benefits click here CLASSIFICATION TITLE: Case Manager - Assessment & Referral SALARY: $50,007.10 - $64,680.01 DEPARTMENT: Community Mental Health Opening Date: 05/27/2026 Closing Date: 06/10/2026 12:00 a.m. **This position is located at Macomb County Community Mental Health North - Assessment & Referral and works primarily as a hospital liaison. FLSA STATUS: Non-exempt - overtime pursuant to Collective Bargaining Agreement EMPLOYMENT RELATIONSHIP: Just cause subject to probationary period CURRENT HOURS AND STARTING TIME: Full-time (37.5 hours per week) position. The start time for this position is currently 8:30 a.m. GENERAL SUMMARY: Under direction, provides direct service to individuals served, family members, and designated others. Utilizes a Person-Centered process and Family Centered Practice Concepts. Coordinates, consults, and provides liaison work with physical health care providers and other agencies. Implements program goals and activities in keeping with program philosophy. Contributes expertise from professional discipline to team planning and treatment processes. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: Performs direct clinical services, including, telephone screening of referrals, assessments of adults, crisis intervention, treatment plans, case management, group therapy and short-term treatment interventions. Performs indirect clinical services, including, case documentation following Macomb County Community Mental Health (MCCMH) policies and procedures Participates in case consultation to confirm diagnosis and to make the most suitable, least restrictive clinical recommendations, and assists the individual and/or family in obtaining appropriate programs and resources. Maintains accurate and up-to-date clinical documentation, including assessments, progress notes, treatment plans, and closings. Ensures administrative paperwork is accurate and up to date, including, Staff Activity Logs, statistical reports, attendance, mileage reports, and case lists. Completes and maintains MCCMH and Department of Mental Health statistical reports. Participates in agency and system staff meetings, and jail diversion and/or court meetings. Maintains appropriate contact with hospitals, agencies, and other service providers to coordinate the care and referrals of individuals served, and acts as liaison for the individual’s needs. Requests appropriate authorizations for services provided; ensures service authorizations are current, monitors utilization of services, and requests re-authorization or continuations. Serves on MCCMH or community-based committees. Measures the effectiveness of interventions to determine outcomes and identify the needs of the person served. Operates an automobile while performing assigned job duties. Performs related duties as assigned. QUALIFICATIONS: Required Education and Experience Bachelor’s Degree from an accredited college or university in Psychology, Social Work, Special Education, Counseling or other mental health related field Must meet qualifications as specified in the Michigan Department of Health and Human Services (MDHHS) Prepaid Inpatient Health Plan (PIHP)/Community Mental Health Services Programs (CMHSP) Qualifications per Medicaid Services as a Qualified Mental Health Professional (QMHP); 1) Individual with specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) or one (1) year of experience in treating or working with a person who has mental illness; and is a psychologist, physician, educator (with a degree in education from an accredited program), social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, registered nurse, therapeutic recreation specialist, licensed or limited licensed professional counselor licensed/limited-licensed professional counselor, licensed or limited licensed marriage and family therapist, or a licensed physician’s assistant; or 2) A human services professional with at least a bachelor’s degree in a human services field Preferred Education and Experience Master’s Degree from an accredited college or university in Psychology, Social Work, Special Education, Counseling or other mental health related field A minimum of one (1) year experience working in a behavioral health setting Required Licenses or Certifications Possession of required state licensure appropriate to position and ability to meet privileging and credentialing guidelines Possession of a valid Michigan driver’s license and operable, insured automobile for authorized travel Preferred Licenses or Certifications None COMPETENCIES FOR SUCCESSFUL PERFORMANCE OF JOB DUTIES: Knowledge of: Applicable computer applications Michigan Mental Health Code and its rules and regulations Various disciplines and assessment tools utilized by each discipline Working understanding on practices for adherence to Medical Services Administration (MSA) Medicaid policies and procedures Techniques to de-escalate/interact in potentially dangerous situations Skill in: Using good professional judgment for the welfare and safety of our persons served Excellent organizational skills with the ability to manage multiple priorities Analyzing information and completing reports Ability to: Collaborative: Approaches all situations with a teamwork and solution focused mindset demonstrated through active participation in meetings, volunteering to assist teammates and other departments as appropriate.
