Lead CNA - Day Center

Job Summary: 

The lead CNA position reports to the Day Center Director and is responsible for establishing leadership in the PACE center, provides organization and direction to floor staff daily by coordinating CNA assignments, mentoring and training of new staff, attendance in IDT meetings and sharing information obtained with floor staff.  In addition, ensures completion of all CNA assignments and coordinates care with the health center.  Collaborates with the Day Center Director daily regarding operations, staff education and care plan contributions. 

The Lead CNA shares information obtained during IDT meetings with the Day Center Nurse and collaborates with the nurse to encourage CNAs to report to the nurse participants’ concerns or changes in condition. The Lead CNA ensures that nursing recommendations such as elevating lower extremities, toileting schedules, offering fluids on as scheduled, etc... are completed.

Job Duties/Responsibilities:

* Ensures coordination of services in order to provide a calm, even flow of the PACE center by assigning staff to appropriate work assignments and               rotating staff fairly. * Creates work assignments for CNA staff and ensures their completion at the end of the shift.  Reports discrepancies to the Day Center Director. * Immediately reports any high-risk issues to the Day Center Director and other issues requiring attention. * Works to maintain an appropriate and positive atmosphere in the PACE center, enforces the Declaration of Cooperation among staff and participants. * Ensures the timely completion of VS, blood sugars, heights and weights as requested by clinical team and are recorded in the EHR. * Serves as a mentor and trains new CNA employees as well as provides on-going support to established staff.  Assists in the interview process for new       staff as appropriate and contributes to CNA performance evaluations yearly. * Completes new staff competencies with the assistance of nursing and ensures a thorough orientation.  Reports any concerns or training needs to supervisor.  * Coordinates with the Life Enrichment staff to assist and support as needed to ensure appropriate attendance and participation in events and                       encourages socialization opportunities. * Works with the Day Center RN to identify the learning needs of the PACE center staff or in-service opportunities for the Organization to reduce risk or increase CNA competency and skills. * Monitors food service for appropriateness of client groupings during meals to ensure safety and socialization. * Provides behavioral supervision and support for participants, monitors and responds to behavioral disturbances such as wandering, agitation, restlessness, exit seeking, etc. and enforces behavioral plans. * Has a current CNA and CPR certification, strong leadership skills and ability to mentor new staff.  Polite, well-groomed, organized and capable of handling multiple assignments at once. * Ensures infectious disease control procedures are adhered to (such as sanitizing the day center) and CNAs are wearing proper PPE at all times. * Report any facility improvements or damaged items in need of replacement to the Day Center Director.  Formulate a plan for staff to work without item         while being repaired if necessary.  * Provide assistance to management staff by providing support to staff, information regarding clients and day to day operational issues, participating in IDT meetings, reporting high risk issues immediately and maintaining a sense of order and flow in the PACE center * Completes the day center assessment tool for new and existing participants and presents findings in care planning meetings * Maintain on-going and accurate participant lists for showers, toileting, diet, allergies and attendance according to participants care plan.  Ensure accuracy of service summary in EMR. * Coordinates assistance with the Health Center for any medical or consult visits as requested.  Provides staffing to health center when needed.  * Problem solves staffing issues between transportation, day center and health center and assigns staff to different departments as needed per skill level. Ensures CNA competency to work in the assigned area.  * Ensures the completion of the daily attendance sheet and assist in making daily calls when requested to ensure participants safety when call outs are received by reception.  Ensure communication to the IDT and RN * Performs all other duties assigned by the Day Center Director.

Required Skills & Abilities:  Ability to maintain confidentiality Dependable and punctual with ability to maintain consistent attendance Ability to read, write and comprehend English Ability to maintain sound judgement under stress and communicate effectively Ability to proficiently perform clinical/skilled tasks associated within scope of position Proficiency with MS Office Suite or similar software 

Education Requirement:  High School Diploma or Equivalent, Required

Physical Requirements:  Physical ability to regularly stoop, bend, kneel, lift, stand, walk, stretch and reach for extended periods of time. Must be able to regularly lift objects up to 25 pounds and at times up to 35 pounds. Must be able to assist a 200-250 lb participant, using proper body mechanics. Must have hearing and vision abilities within normal range (corrected) to drive safely and/or to observe and communicate with participants. Must be able to navigate various departments of the organization's physical premises as well as related community care settings. Must be able to tolerate conditions typically associated within a medical office and/or home care setting including potential exposure to bloodborne pathogens and infectious diseases. Must be able to complete assignments in variable weather conditions and ever-changing and sometimes allergenic environments in participant homes, including extreme heat, humidity, dust, smoke, pet contamination, and cleaning supplies. 

Experience:   3 years of CNA experience, Required 1 year of experience working with a frail or elderly population, Preferred 

Licensure & Certification Requirements:  Rhode Island Certified Nursing Assistant License (CNA) 

Position Requirements: CPR Certification Driver's License & access to reliable transportation  

Monday - Friday 7:00am-3:30pm
