Certified Nursing Assistant (CNA) – Home Health & Hospice Ascend Health is looking for a compassionate and dedicated Certified Nursing Assistant (CNA) to join our Home Health and Hospice team. As a CNA with Ascend, you will play a vital role in providing hands-on care and support to patients and families in the comfort of their homes. Key Responsibilities: Provide personal care, companionship, and support to patients in their homes. Assist with activities of daily living (bathing, dressing, grooming, feeding, mobility, toileting). Monitor and report patient condition changes to the supervising nurse. Provide emotional support to patients and families. Document all patient care and maintain accurate, timely records in the EMR. Follow the plan of care as directed by the clinical team. Qualifications: Current CNA certification in the state of Idaho (or ability to obtain). Previous experience in home health, hospice, or patient care preferred. Compassionate, patient, and reliable with strong communication skills.
