JOB TITLE: LICENSED PRACTICAL NURSE (LPN) – HOME HEALTH
POSITION SUMMARY
The Licensed Practical Nurse (LPN) provides direct, hands‑on nursing care to patients in their homes under the supervision of a Registered Nurse (RN) or physician. The LPN supports patient independence, safety, and comfort while following individualized care plans and agency policies.

ESSENTIAL DUTIES AND RESPONSIBILITIES
* Provide skilled nursing care to patients in their homes according to the plan of care * Monitor and document vital signs, symptoms, and overall patient condition * Administer medications and treatments as prescribed * Perform wound care, catheter care, ostomy care, and dressing changes * Assist patients with activities of daily living (ADLs) as needed * Observe and report changes in patient condition to the supervising RN or physician * Provide education to patients and caregivers on disease management, medication use, and safety * Maintain accurate, timely clinical documentation in accordance with regulatory requirements * Follow infection control, safety, and confidentiality standards * Collaborate with interdisciplinary team members to ensure coordinated care * Participate in case conferences, in‑service training, and quality improvement activities

QUALIFICATIONS
Education and Licensure
* Graduate of an approved Practical Nursing program * Current LPN license in the state of practice * CPR certification (BLS)
Experience
* Previous home health or clinical nursing experience preferred * Experience with geriatric or chronic care patients a plus

* Knowledge of basic nursing principles and home health practices * Ability to work independently and make appropriate clinical decisions * Strong assessment, communication, and documentation skills * Compassionate, patient‑centered approach to care * Ability to follow care plans and physician orders accurately * Basic computer skills for electronic health records * Reliable transportation and ability to travel to patient homes

WORKING CONDITIONS
* Work performed in patients’ homes and community settings * May require travel, flexible scheduling, evenings, weekends, or holidays * Ability to lift, reposition, and assist patients as needed
