Job Title: Case Manager Bilingual Division/Program: Adult Full-Service Partnership Starting Compensation: 22.00 - 28.00 USD Per Hour Working Location: Community Based Fresno County Working Hours/Shift: Monday-Friday, 8am to 5pm. Employee Recognition & Rewards: A culture that celebrates and rewards your hard work and dedication What you bring to SBHG: Associate degree plus two (2) years of experience working in a mental/behavioral health field required (Internship may be substituted for a portion of the work experience) OR Bachelor’s degree plus six (6) months of experience preferred. (Internship may be substituted for a portion of the work experience) Valid California Driver’s License required and must meet Agency driver eligibility criteria. How you will make a difference: Work within the program model’s framework, including appropriate Evidence-Based Practices, to provide services to clients and their families. Provides services at the office, in the community, and in clients’ homes. Uses a personal vehicle to drive within the community to provide services and to transport clients in the personal vehicle. Provides case management services to clients and their families, as deemed necessary and appropriate through the assessment process and in collaboration with the treatment team. Participates in meetings, such as Treatment Team Meetings, to ensure clients’ needs are met and recommendations are followed through. Documents all services provided clearly and concisely within the Electronic Medical Record within the timelines and framework established by Stars. Provides services in the community, and in client’s homes. Uses personal vehicle to drive within the community to provide services and to transport client and/or client’s family members in personal vehicle.
