Position Summary
Coordinates and provides pharmaceutical care for patients, collaborating with the interprofessional team to optimize safe, effective medication use. Reviews and verifies orders, manages drug therapy protocols, educates staff/patients, and supports quality and regulatory standards. Reports to the Director or Manager of Department.
Essential Functions
* Manages drug therapy protocols as part of an interdisciplinary team. * Reviews, interprets, and evaluates medication orders to identify, prevent, and resolve drug-related problems. * Provides drug information and education to medical staff, healthcare professionals, patients, and pharmacy students. * Documents interventions and outcomes for drug therapy quality improvement. * Verifies medication orders for accuracy/completeness prior to entry; checks filled medications prepared by technicians; maintains proficiency with pharmacy automated systems and software. * Manages patients in an outpatient anticoagulation clinic, including point-of-care testing, result evaluation, dosing, and scheduling. * Participates in performance improvement and required continuing education. * Performs other duties as assigned.

* Education: Doctor of Pharmacy * License: Current license with the state Board of Pharmacy * Certifications: BCLS required
