The Department of Mental Health (DMH) is seeking dedicated, compassionate, and exceptional Registered Nurse who desire a rewarding career making a difference in the lives of individuals in the community. The Registered Nurse II serves as a member of the Adult Community Clinical Services (ACCS) Integrated Team; provides behavioral and physical health monitoring and support; Medication Administration Program oversite, coordination, and education; completes PHAs; participates in the development of IAPs; provides trauma-informed, person centered community-based interventions that target skill development in the areas of Health & Wellness; conducts routine assessments and provides medical care as needed; ensures that appropriate medical services are accessed by collaborating with care coordination entities. The selected candidate will function as the ACCS Team RN providing nursing oversight, consultation, and problem solving to the team’s LPNs and MAP Certified team members. Please Note: This is a 40 hour per week position for the first shift, working 8:30 am to 5:00 pm, with Sunday and Saturday off. Employees in this position will be required to perform mandatory overtime. Duties and Responsibilities (these are a general summary and not all inclusive): Ensures meeting of MAP standards through education, coordination and oversight implementation and QA reviews of MAP process. Provides initial MAP testing to new hires/transfers, provides recertification training and testing for existing staff, conducts regular MAP inspections of all DPH registered sites. Serves on a ACCS Integrated team by attending regularly scheduled meetings, by coming prepared to present psychosocial history, mental status, diagnosis, treatment provided, and the treatment planned for assigned cases and by participating in discussion of other cases so as to ensure that persons served receive optimal care. Assists in the development, understanding, & implementation of lAPs of those that are on assigned case load by involving person served and significant others as appropriate. Assess on an ongoing basis the needs of assigned persons served by providing supportive counseling, by reviewing records, by making home visits and by maintaining regular contact with persons served and families in order to ensure that appropriate recommendations for treatment are made. Functions as an advocate and liaison for persons served in interactions with the person served primary and other health care providers, conferring as necessary regarding medical and medication issues and adjustments. Provide rehabilitative interventions to assist persons served in building and using skills and accessing resources to achieve treatment goals, manage symptoms and live productive and meaningful lives. These interventions take into account SAMHSA’s 8 Dimensions of Wellness. Read & communicate all pertinent information and attending daily rounds, staff meetings and informing supervisor or designee of any critical information. Transport or arrange transportation for persons served that need that level of service. Assess persons served mental status and assisting them with utilizing coping strategies, natural supports, and safety planning when necessary. Maintain person served records in a timely manner by documenting in EMR and paper record and completing all other related forms to ensure compliance with DMH regulations and Rehab Option requirements. Provides primary nursing care by assessing health status, recording related health data, evaluating outcomes, and adjusting care in conjunction with other service providers. Communicate pertinent information to ACCS staff, as well as other treatment providers and care coordination entities, to ensure integration, coordination, and continuity of care. Transcribes and ensures implementation of physician’s orders. Conducts self-medication readiness assessments at the time of admission and during care transitions. Completes physical health assessments and verifies medications at time of admission and update as needed. Provide education and support regarding the importance of obtaining annual PEs and assist with the coordination of obtaining annual physical exams, routine medical and dental consultations and all health care services and accompanies persons served when needed. Assists staff in meeting MAP standards and provides education and supports as needed. Leads or co-leads groups in the area of Health & Wellness. Required Qualifications: Knowledge of the principles and practices of nursing. Knowledge of the terminology and standard abbreviations used in nursing. Knowledge of the problems in providing nursing services to special client groups such as the mentally, physically, and/or emotionally handicapped. Knowledge of standard nursing policies, procedures, methods and techniques. Preferred Qualifications: Certification as Psychiatric Nurse by American Nurse Association. Knowledge of Medication Administration Program. Experience working with individuals with mental illness and co-occurring disorders. Experience working in a 24/7 residential program and community setting for adults with mental illness. Given population served, bilingual or multilingual candidates.
