🧾 Position Overview
We are seeking a compassionate, motivated, and clinically skilled Nurse Practitioner (NP) to join our growing primary care practice on a full-time or part-time basis. The ideal candidate will provide comprehensive outpatient care to adult and/or pediatric patients while collaborating with physicians and clinical staff to deliver high-quality, patient-centered care.
This position is ideal for a provider who is comfortable working in a fast-paced environment and values preventative medicine, chronic disease management, and continuity of care.
🩺 Responsibilities 👩⚕️ Clinical Duties
Perform comprehensive patient evaluations, physical examinations, and medical histories Diagnose and treat acute and chronic medical conditions Develop and implement treatment plans Order, interpret, and review laboratory and diagnostic testing Prescribe medications and therapies in accordance with New York State regulations Provide preventative care, wellness visits, and patient education Manage chronic conditions including: Hypertension Type 2 Diabetes Obesity Hyperlipidemia Perform documentation accurately and efficiently within EMR/EHR systems 📋 Administrative & Team Collaboration
Collaborate with physicians, medical assistants, and administrative staff to ensure smooth patient flow Maintain timely chart completion and documentation compliance Participate in quality improvement and patient satisfaction initiatives Assist with medication prior authorizations and care coordination when needed ✅ Qualifications 🎓 Education & Licensure
Master’s or Doctorate degree from an accredited Nurse Practitioner program Active New York State Nurse Practitioner license Board certification as: Family Nurse Practitioner (FNP), or Adult-Gerontology Primary Care NP (AGPCNP) Active DEA registration (or eligibility) 🧠 Experience
Minimum 1 year Primary care experience preferred (new graduates may be considered depending on training and background) Experience with outpatient EMR/EHR systems Strong communication and patient education skills Bilingual candidates encouraged to apply (Spanish, Portuguese, etc.) 💡 Preferred Skills Experience with: Annual wellness visits Weight management Preventative medicine Women’s health Men’s health Ability to work independently while collaborating with supervising physicians Strong organizational and multitasking abilities ⏰ Schedule Options Full-Time
