We are seeking a dedicated Part-Time Hospice Social Worker (MSW) to provide individualized hospice support services at the Kate B. Reynolds Hospice Home.

SCHEDULE

Part-Time – 20 hours per week

Available schedule options include:

* Saturday / Tuesday / Thursday OR * Sunday / Wednesday / Friday

Additional scheduling options may be discussed, though one weekend day is required.

POSITION REQUIREMENTS

✔ Master’s Degree in Social Work (MSW) required ✔ Knowledge of community social, financial, and healthcare resources ✔ Strong written and verbal communication skills ✔ Ability to work independently and within an interdisciplinary team ✔ Commitment to hospice philosophy and compassionate care ✔ Excellent organizational and prioritization skills

KEY RESPONSIBILITIES

As a Hospice Social Worker, you will:

* Provide individualized hospice services to patients and families * Assess emotional, social, and financial needs * Develop psychosocial goals within patient care plans * Support patients and families in accessing community resources * Collaborate closely with the interdisciplinary team (IDT) * Attend and contribute to IDT meetings * Assist with discharge planning and transitions of care * Educate families regarding Hospice Medicare/Medicaid benefits * Complete required clinical documentation accurately and timely * Support orientation of new staff, interns, and volunteers * Participate in Social Work on-call duties
