Responsibilities - Administer prescribed medications and treatments according to physician orders and policy - Monitor patient status, record vital signs, and report changes to the supervising nurse - Perform wound care, dressing changes, and support rehabilitation activities as directed - Assist patients with activities of daily living and mobility, while promoting safety and dignity - Document care provided and maintain accurate, timely nursing notes; adhere to care plans and facility policies - Collaborate with the registered nurse and interdisciplinary team to ensure coordinated, high-quality care
Requirements - Active Licensed Practical Nurse license in Pennsylvania - Availability for evening shifts and a 40-hour week on a travel assignment - Ability to commit to an 11-week contract - Prior long-term care or skilled nursing experience preferred - Strong communication, observation, and documentation skills; ability to work with an interdisciplinary team - Willingness to undergo standard background check and drug screen as required
