About the Role:
As a Pharmacist, you will play a crucial role in ensuring the safe and effective use of medications for patients across various healthcare settings. Your primary responsibility will be to evaluate and manage medication therapy, providing expert advice to healthcare professionals and patients alike. You will also be involved in pharmacoeconomic assessments to optimize medication costs while maintaining high-quality care. Additionally, you will educate patients on medication usage, potential side effects, and the importance of adherence to prescribed therapies. Ultimately, your expertise will contribute to improved patient outcomes and the overall efficiency of healthcare delivery.
Minimum Qualifications:
* Doctor of Pharmacy (Pharm.D.) degree from an accredited institution. * Active pharmacist license in the state of practice. * Completion of an accredited pharmacy residency program or equivalent experience.
Preferred Qualifications:
* Board certification in a pharmacy specialty (e.g., BCPS, BCGP). * Experience in clinical pharmacy or hospital pharmacy settings. * Familiarity with pharmacy benefit management systems. * Experience in a compounding setting.
Responsibilities:
* Review and interpret medication orders to ensure accuracy and appropriateness. * Provide medication therapy management and patient counseling to optimize therapeutic outcomes. * Conduct immunization services and educate patients on vaccine benefits and schedules. * Collaborate with healthcare teams to develop and implement patient care plans. * Monitor patient responses to medications and make necessary adjustments in therapy. * Other task assigned
