See what it's like to work for KeystoneCare: https://www.youtube.com/watch?v=DyEH9buz_Vc KeystoneCare is an Equal Opportunity Employer Primary Job Responsibilities: Supervise and support nursing and clinical staff, ensuring quality and consistency of patient care Review and approve patient care plans, documentation, and clinical records for accuracy and compliance Provide coaching, training, and performance feedback to clinical team members Ensure compliance with state, federal, and accreditation standards (e.g., Medicare, CHAP) Assist with case management, scheduling coordination, and patient assignments as needed Collaborate with interdisciplinary teams to ensure coordinated and effective patient care Participate in quality improvement initiatives and clinical audits Serve as a clinical resource for staff, patients, and families Support onboarding and orientation of new clinical staff Accepts direction from CEO and VP of Clinical Services. Maintain confidentiality with regard to patient information Performs other duties as assigned 3 + years' experience with a healthcare organization; preferably in acute care, home health or hospice environment preferred. RN License. Must be able to multi-task in a fast-paced environment and communicate effectively. Excellent customer service skills and proven ability to work with healthcare professionals, patients and their families responding to time-sensitive needs Excellent data entry skills Dedicated self-starter who can work both independently as well as collaboratively as part of a team of healthcare professionals. Proven technology skills and experience using an electronic documentation system. Proficient in Microsoft computer products including Word, Excel and Outlook. Qualifications Active and unrestricted RN license in Pennsylvania (required) Minimum 2–5 years of clinical experience in home health, hospice, or palliative care Prior supervisory or leadership experience preferred Strong knowledge of Medicare/Medicaid regulations and documentation requirements Excellent communication, leadership, and organizational skills Ability to manage multiple priorities in a fast-paced environment Accrued Paid Time Off Paid Holidays Medical, Dental, Vision, and Aflac plans available $25K Company Paid Life Insurance Policy Retirement plan with discretionary employer match
