As a Nurse Practitioner supporting Home Health, Hospice, and Palliative Care, you will:
* Conduct patient visits across all three service lines * Collaborate and participate in weekly interdisciplinary group (IDG) meetings * Provide clinical guidance and continuity of care * Play a key role in shaping patient-centered outcomes

* Flexible, mission-driven work environment * 4 weeks per year: Monday–Monday coverage rotation

Requirements:
* Graduate of an approved Nurse Practitioner curriculum, MSN required * Active APRN or NP license required * Minimum of two years clinical nursing experience * Valid driver’s license
